Registration, Waiver & Disclaimer Form

  Spring Season 2010

1.Player agrees that he/she is 18 years or older and has shown proof of age.

2.Player agrees to pay the full Registration $75.00 fee before being allowed to play.

3.Player agrees that there are no refunds.

4.Player agrees to follow all the league rules at all times.

5.Player agrees that he/she has read all the rules. (Rules located at www.yvasladultsoccer.com)

6.Player agrees not to engage in fighting, un-sportsman like contact, violent or unethical acts or will to be removed from the               League with no refund.

7.Player understands that in any case of a returned check, he or she will be charged a $25.00 NSF Charge.

8.Player agrees that the registration fee does not cover uniforms.

9.Player and teams agree that they are responsible for their own uniforms.

10.      Player acknowledges that YVASL is not responsible for any injuries, bodily harm or death to the player at anytime.

11.      Player agrees to hold YVASL, all of YVASL Employees, The City of Beaumont and The County of Riverside harmless in the event of any injury suffered to the player before, during and after a game.
